PURPOSE OF THE POST
The post will involve leading out, managing and implementing a reform
initiative that is part of a Service Reform Programme within
Carndonagh region funded through the Service Reform Fund (Genio). The
Service Reform Fund (SRF) has been established to support the
implementation of reforms in the Disability Service in Ireland. The
SRF provides resources to re-configure existing services towards more
person-centred, cost effective approaches in line with government
policy.
The post holder will be responsible for the coordination of the
transition of service users attending JCM Carndonagh and Milltown
House to a modernised, person-centred service that is aimed towards
community inclusion and active citizenship in line with New Directions
policy. Service users are due to move from a congregated setting to
homes in the community and therefore an ideal time to review their day
service provision, their PCPs and explore how they could be
integrating more with activities within their new community.
The post will require mentoring and capacity building amongst staff,
families and services users as well as conducting regular review
procedures with management to ensure a collaborative approach and a
smooth transition for all parties involved in the process of
implementing this new model of service delivery.
The Transition Coordinator will also provide support and guidance to
staff in the establishment of natural supports, development of valued
social roles and the formation of circles of support for service users
with minimal support needs aiming towards the reducing reliance on
paid staff and time spent in the day service.
